To change the name on your Social Security card you are required to contact the Social Security Administration (SSA). Replacing your Social Security card because of a name change is a common practice; newlyweds and divorcees do it frequently. But if you change your name for a reason other than marriage or divorce, you will need a court order to obtain permission to legally change your name. Once you have legal documentation your new birth certificate, a marriage license or a divorce decree you can easily change your name on your Social Security Card.

      Locate a copy of form SS-5, which is the application for a replacement Social Security card from the SSA. Pick one up at your local SSA office, or download and print one from the SSA website. Locate your nearest SSA office by using the SSA local office search page - enter your zip code and view a list of every SSA office in your city.

      Complete form SS-5 by entering your name at birth, as it currently appears on your Social Security card, and how it should appear on your new card. Include your address, Social Security number, and date of birth.

      Gather your supporting documents to submit with your replacement card application. You must provide proof of your identity in the form of photo identification as well as supporting documentation stating your name change. In this instance you must provide your new birth certificate and the court order that states the reason for your name change on your birth certificate.

      Deliver your application and supporting documents to your local Social Security office. Once you present your application and information to the SSA office you will receive your new Social Security card through the mail. This typically takes one to two weeks.

      Mail your application to your local Social Security office. You must mail your application and supporting documents, and allow up to two weeks to receive your new card.

Tips & Warnings

  • Your photo identification, birth certificate and proof of name change must all be original or certified copies. Photocopies are not accepted by the SSA.
