Of course you can steal these surveys from friends or other websites, but with a little creativity you can create your own survey for everyone to fill out. Think of a theme for your blank fill in survey. The theme is what you will base all of your questions around. For example choose a theme like "love" and ask questions about a current or past love.

Decide what style of questions you want to ask. You can use questions with short answer or with yes or no answers. Think of the complexity of answers you wish to receive from your friends as well as the amount of information you wish to share. A sample open ended question would be "Where did you have your first kiss?" while a yes or no question would be something similar to "Have you ever drank alcohol?"

Decide how many questions you want to ask. The shorter your survey, the more likely people will want to take it.

Type the questionnaire in a word processor and save to your computer. At the beginning or the end of the survey include a statement to read, copy and fill out for your friends. Copy the survey and paste in a blog on MySpace or as a bulletin. Use a subject such as "Love Survey".
