How to Get My Pharmacy Records

You can order your pharmacy record at any time.

Each pharmacy is required to give you access to your pharmacy record. Your pharmacy record contains a list of your past prescriptions. You may need to look over your pharmacy record to verify billing, go over past prescriptions, insurance purposes or just for your own personal information. To protect the privacy of the record owner, pharmacies will require that you have an interest or right to the record.

Instructions

1. Request a copy of your pharmacy records. Some pharmacies will accept the request by phone or in person; others require that you complete a records request form. You must submit the patient's full name, date of birth and address on the form or to the representative.

2. Go to the pharmacy and pick up the record. Some pharmacists will send the record to your home upon request.

3. Submit proof of identity or the right to the record. Right to the record includes guardianship papers, power of attorney or a birth certificate. If sending the request, make a copy of this proof and submit it with the form, by mail. If requesting the record in person, show proof.
