How to Get a Copy of a 1099 Form From 2009
A 1099 is a form that must be completed annually by anyone that paid a contractor more than $600 in a single calendar year. A 1099 must be filed on people paid but not treated as employees by a company as well as for prizes totaling more than $600, such as casino or lottery winnings. If you have misplaced your 2009 1099 form, you should first contact the company or organization that issued it to you. If they are not able to provide it, request a copy from the Internal Revenue Service (IRS).

Contact the employer or company that issued to the 2009 1099 form and request an additional copy.

Download and print form 4506-T from the IRS website.

Write your name, address, social security number and previous address if you have recently moved in boxes one through five on the form.

Place a check beside section eight of part six, indicating that you are requesting copies of form W-2, 1099, 1098 or 5498. Only the forms on file with the IRS will be sent because all four forms may not apply to you.

Write "2009" on the first line of section nine.

Sign and date the document and have your spouse do the same, if applicable.

Locate the section on page two of the 4506-T form labeled "Chart For All Other Transcripts." Locate the state in which you lived when the return was originally filed. Mail the 4506-T form to the address listed to the right of the box containing the state in which you lived when the return was originally filed.
