Instructions for a Massachusetts DOR Form One

Persons who reside in Massachusetts are required to pay income tax on revenue derived from sources in Massachusetts. As of 2011, the income tax rate on income derived in Massachusetts is 5.3 percent. A Massachusetts resident can file his annual tax return by filling out and filing Form 1 with the Massachusetts Department of Revenue.

      Download a copy of Form 1 from the Massachusetts Department of Revenue website (see Resources). The Massachusetts Department of Revenue provides a PDF version of Form 1, which may be printed, filled out and filed.

      Provide your personal information. Provide your full name, your spouse's full name, your Social Security number, your spouse's Social Security number and your address. Indicate any donations you wish to make to the State Election Campaign Fund and your filing status.

      Indicate your income and any exemptions. Provide a personal exemption of $4,400 if single or married filing separately, $6800 if head of household or $8,800 if married filing jointly (exemptions as of 2011). Indicate the number of dependents in your household, the number of individuals over the age of 65 or blind and perform the necessary calculations to determine the total value of your exemptions.

      Indicate any deductions and long-term capital gains. Provide the value of any wages, tips or compensation listed on your W-2. Indicate any income from taxable pensions, annuities, interest from Massachusetts bank accounts, business income or losses, unemployment compensation, state lottery winnings and other income. On line 10, indicate your total income. Indicate any amount paid to Social Security, Medicare or retirement, expenses incurred in providing care to a child under the age of 13, the number of dependents, and any amount paid toward renting property. On line 16, indicate the total value of your deductions. On lines 17 through 27, perform the requested calculations.

      Indicate any tax credits. On lines 28 through 35, perform the requested calculations to determine your income tax after credits.

      Calculate any payment due. On lines 36 through 45, indicate any prior tax payments made during the past three years and perform the requested calculations. On line 46 and 57, perform the requested calculation to determine the amount of your refund or tax payment. Provide the routing number and account number for your checking or savings account for direct deposit.

      Sign and file Form 1 with the Massachusetts Department of Revenue. On page 1, sign and date the form. The mailing address for filing, which may be found in the Instructions for Form 1 (see Resources), depends on whether you owe a payment.
