1. Download a copy of the Toledo Police Department Report Request Form (see Resources). The Toledo Police Department provides an online copy of their report request form, which can be printed, filled out and filed to obtain a copy of your criminal record.

2. Fill out the request form. Provide your name and mailing address. Provide the full name, known addresses, date of birth, Social Security number, race, sex, height and weight of the person whose criminal record you want a copy of. Provide your name, address and signature.

3. Mail your request form to the Toledo Police Department at Criminal Justice Coord. Council; NORIS Division; 1 Gov't Center, Suite 1720, Toledo, Ohio 43604. As of April 2011, the fee for obtaining a report from the Toledo Police Department is $8, which must be paid by check made payable to the City of Toledo Police Department.
