Instructions

1. Visit your local Costco store.

2. Go to the membership services desk -- this is likely the same place where you purchased your membership.

3. Present your membership card and photo ID. Explain to the customer service representative that you are dissatisfied with your membership and, in accordance with company policy, would like a refund for your membership fee.
