Changing your name on your Social Security card is not difficult.

Marriage alone does not automatically change a woman's legal last name in Kansas. However, marriage entitles a woman to change her last name without petitioning a Kansas court. In order to change her last name, the married woman must notify several agencies of her marriage by presenting her marriage certificate and letting them know about her new name. One such agency is the Social Security Administration. Women who get married in Kansas may change their last name with the Social Security Administration in person or in writing.

      Navigate to the Social Security Online website. Click on "Get or replace a Social Security card." Download and print form SS-5, "Application for a Social Security Card."

      Complete the application using black or blue ink. In box 1, write the full name that you would like listed on your new Social Security card. Below that, write your full name as it is listed on your birth certificate. Enter your Social Security number in box 2. In boxes 3 and 4, respectively, write in your place and date of birth. Indicate your United States citizenship status by checking a box in box 5. If you would like, you may indicate your race and ethnicity in boxes 6 and 7, respectively. Check the male or female box in box 8. In box 9, list your mother's name as it was listed on her birth certificate and her Social Security number. Next, in box 10, do the same using your father's name and Social Security number. In box 11, indicate whether you have ever had a Social Security number card and, in box 12, write down the name that appears on your most recent Social Security card. Write down the date, your phone number and mailing address in boxes 14, 15 and 16. Finally, sign the form.

      If you do not have a copy of it, retrieve a certified copy of your marriage certificate from the Kansas county in which you were married.

      Find your local Security Security office by navigating to the Social Security Office Locator on your web browser. Bring the completed application and your marriage certificate to your local Social Security office or mail both documents to that office.

      Wait for your new Social Security number card to arrive in the mail. Destroy your old Social Security number card.
