How to Write a Survey Form
Survey forms are important tools used by many types of organizations. The primary goal of a survey is to obtain useful feedback needed to make important business decisions. When writing a survey form, it is essential to first determine what information you are trying to obtain through this activity. Surveys generally are used to gain customer evaluations of products, customer opinions and to locate market trends.

Define your survey goals. Before writing a survey, the first step is determining the survey's primary purpose. Organizations creating a survey should investigate what they hope to learn from this and base the survey around this. Create clear objectives for the survey prior to writing the questions that will be used in it.

Determine who will participate in the survey. A company should decide who will receive the survey. Many surveys are distributed to current customers, potential customers or employees.

Plan the questions that will be asked. Begin with demographical questions regarding marital status, age group and income categories. Proceed to questions that specifically relate to the objectives of the survey. Write clear questions that are easy to understand.

Organize the questions in a logical order. Do not jump from product quality to price-related questions and then back again to product quality. Keep like questions grouped together and organize the questions in the most systematic approach as possible.

Choose an answer system. Multiple choice questions are commonly used because they are tallied easily. A rating system, where customers are asked to rate certain things using a scale, is also commonly used. A common scale is a 1 through 5 scale, where "1" represents "dissatisfied" and "5" represents "extremely satisfied."

Leave a place for comments. Leave an area at the end of the survey for the person to comment on anything he feels necessary.

Include additional details. Explain at the beginning of the survey what the purpose of the survey is and give an estimation of how long the survey will take to complete. Include how the survey should be returned and the deadline date.
