Create a scale basis for your questions to measure customer satisfaction. For example, use a satisfaction scale of 1 to 10 where 1 means "Very Unsatisfied" and 10 means "Very Satisfied" for certain questions. Use an expectations scale where 1 is "Expectations Not Met At All" and 10 is "Expectations Exceeded" for other questions.

Query customers about their satisfaction with your employees.

Ask customers about their satisfaction with your products, if relevant. Have customers include a UPC code from the products with their survey.

Ask about satisfaction with the cleanliness of your place of business.

Ask follow-up questions to avoid vague responses. For example, when asking about customer satisfaction regarding a product, follow up with "yes" or "no" questions to determine why a customer was not satisfied.

Include a "Not Applicable" option so you are not asking a question that may be irrelevant to some of your survey participants.

Ask all questions in a clear and precise manner so customers cannot misinterpret them.

Include a section where customers rate the importance from 1 to 10 of factors like price, product quality, deliver time and customer service. Have 1 stand for "Not Important At All" and 10 for "Very Important."
