Prospective employers are among those who ask for detailed past employment records.

A prospective employer may ask to see a report of your job history to confirm an item noted on your resume, or to verify certain details about your former employers for reference. Only an HR director who is interested in hiring you usually makes such a request. Other situations requiring information from your past employment records include court cases for personal injury and employment verification for pension plans. The Social Security Administration has complete employment records available for a fee.

      Print and fill out Social Security Form 7050-F4 with your name, any former name such as a maiden name, Social Security number, date of birth and the years of past employment records requested (see Resources). Indicate if the job history needs to be detailed or certified by checking the boxes for such. Detailed history requests require an explanation as to why the information is needed.

      Sign the form to verify your identification stating that you're requesting information about yourself or someone who authorized you to do so. Write down an address for the administration to send the past employment records to.

      Determine your fee for the list of former employers by using the table on Page 3 of Form 7050-F4 for the cost per number of years requested. Add another $15 to the fee, as of February 2011, if you're requesting certified information, as may be necessary for a court case. Detailed job-history reports to correct mistakes on record, as well as requests for yearly totals to show benefits accrued, are free.

      Make payment by including a check or money order payable to the Social Security Administration with your form, or by notating credit card information in the provided section and signing the form for the charge.

      Mail the form to the Social Security Administration post office box address listed, or send it by private courier service to the physical address that receives packages from FedEx, UPS and others. Response time for the request may be as long as four months.
