How to File Medicare Claims in Indiana
Medicare is a federal program that is administered according to federal guidelines. This means that the procedures don't change, no matter what state you're filing from. Call 800-633-4227. Enter your Medicare number when prompted. Ask for the expiration date on the type of claim you're filing. Ask for the address of the carrier who processes the type of claim you're filing in Indiana.

Fill out the Patient's Request for Medical Payment form. Write in your Medicare identification number, your personal identification and address information.
