How to File Medicare Claims in Indiana
Medicare is a federal program that is administered according to federal guidelines. This means that the procedures don't change, no matter what state you're filing from. Indiana's Medicare program is locally governed in Indianapolis, but claims made through Medicare from Indiana must be filed using the federal 800 number set up to assist Medicare recipients. Medicare claims are usually filed by doctors, nurses and other health care professionals. Only file your own claim if you are being billed for a service or procedure and the deadline for Medicare payment is close to passing. Before filing your claim, ask your health care professional to file a claim through her office --- doctors are used to working with Medicare and are supposed to file claims for you.

Call 800-633-4227. Enter your Medicare number when prompted. Ask for the expiration date on the type of claim you're filing. Ask for the address of the carrier who processes the type of claim you're filing in Indiana.

Fill out the Patient's Request for Medical Payment form. Write in your Medicare identification number, your personal identification and address information.

Describe the procedure or service for which you're filing a claim. Write in any information relating to other health care or insurance plans.

Contact your health care provider. Request an itemized bill that includes everything for which you're filing a claim.

Send the form and the itemized bill to the carrier. A Medicare representative should have given you the address when you called. If you don't have it, call again and explain your type of claim. Tell her that you are an Indiana resident so that your claim gets to the correct office.
