How to File Proof of Claim on Bankruptcy Chapter 11 Papers
Businesses often struggle in tough economic times. Consumers aim to save money and spend less time and less money in places of business. As a result, some business owners begin to have a difficult time making ends meet. These business still have expenses and bills to pay on less income than when the economy is booming. This could force the owners of a business to file for bankruptcy. If you have provided products or services to a business who has filed for bankruptcy, and you have not been paid for those products or services, you should file a proof of claim with the bankruptcy court.

Obtain a Proof of Claim form (see "Resources"). The U.S. Courts website provides all the forms needed in connection with a bankruptcy case. The form should be labeled "B 10." Download the form.

Fill in the form. Fill in the name of the judicial district in which the bankruptcy case was filed, the debtor's name and the debtor's case number. Fill in your name, address, telephone number and the account number associated with the debtor.

State the amount you are claiming the debtor owes you. Also state the basis of the claim, the account number or the last four digits of the number you use to identify the debtor.

Attach any documents you have providing evidence that the debt exists and any lien securing the debt. If these documents contain sensitive information, redact that information.

Print your name and your title. Sign and date the Proof of Claim form.

Mail the Proof of Claim form to the bankruptcy court where the debtor has filed its Chapter 11 bankruptcy case.
