How to Get on Original Copy of an Oklahoma Birth Certificate
You can obtain a certified copy of a Oklahoma birth certificate through the Oklahoma State Department of Health Vital Records Service. Requests for birth certificate copies can be made by the individual named on the record, the parents of the individual on the record, spouse of the named individual, step parent, adult child or the attorney of the individual listed on the birth certificate.

Open the Oklahoma State Department of Health web page and click "Birth and death certificates."

Click "Birth certificates."

Scroll to and click "Application for search and certified copy of birth certificate."

Complete the birth certificate application. You can complete the application online and print it or you can print the application and manually complete it.

Visit the vital records or health department and submit the application with your proof of identification and your payment for the total number of certificate copies you wish to obtain. Applications submitted in person are provided the same day.
