Obtain a copy of the 1500 claim form. The 1500 claim form is available on a number of government and private health care websites. Conduct a search using an online search engine for "1500 claim form" to obtain it. You can also obtain one in person by visiting your town's government center.

Select the health insurance program that you're claiming from the options at the top of the screen. For example, if you're using Medicare, circle the Medicare box on the top of the form.

Enter information for the person insured by the policy in the corresponding boxes on the form's right side. This includes the I.D. number, name, address, date of birth, employer's name and plan name.

Enter the name of the insured's primary medical caregiver and his I.D. number. Continue by entering information regarding medical visits, procedures and fees associated with health care in the corresponding boxes on the bottom of the form. If hospitalization ever occurred, enter that information in the corresponding boxes.

Obtain signatures from the physician to indicate that she has reviewed the information and agrees that it's correct. The form is not complete unless the physician signs off on it.
