How to Create a Questionnaire for a Class Reunion
While classmates of the past have the opportunity to catch up at class reunions, it is often wise to gather some information prior to this event. The easiest way to do this is by creating and sending out a reunion questionnaire. If you create an effective questionnaire, you can use the information gleaned to prepare for your event and develop a better understanding of who your former classmates are now.

Select a delivery method. Print your questionnaires on paper or use a more high-tech delivery method, such as creating an online survey for classmates to fill out. Doing it online will probably get quicker responses and will save you money in postage costs.

Compose a list of questions. Create a mix of yes and no and response questions. Ask more questions than you feel like you will necessarily include in future information about alumni, as you can always edit out questions but you can’t easily ask questions that you later wish you'd asked.

Send out questionnaires themselves or instructions on how to complete them to reunion guests. If using a paper questionnaire, mail the questionnaire and instructions to the alumni. If using a digital option, you can simply mail a postcard with information printed on it about how to complete the questionnaire.

Allow for easy return. If you have asked reunion attendees to complete the questionnaire online, no return method is needed. If, instead, you are asking them to mail it back to you, include a self-addressed, stamped envelope to ensure that they can do so with ease.
