How to Set Up Templates on Your Computer for Medical Transcriptionist

Open up a new file on a word processing program on your computer. Either a Mac or PC will work, but your transcriptionist must have a compatible system.

Type up the main categories of information gathered during the patient interview. This should include the patient's identifying information, chief complaint, history of present illness, past medical and surgical history, list of medications, allergies, and immunization history.

Insert the categories of information gathered during the physical exam as well as assessment and plan. Include a review of systems section, organized in chart format and divided up based on organ system.

Organize the template in the order you typically gather information from the patient. The transcription process will be less prone to error if your template is organized in the same order that you dictate your notes.
