      Complete Part 1. If you're reporting the change for your individual tax account, check box "1." If you previously filed a joint return, but are now separated from your spouse, check the box that indicates you'll have an address different from that of your spouse.

      Provide your old address and new address. The IRS will verify the change is made to the correct account by using your old address and Social Security number as matching items.

      Sign the form. If you're married and file jointly, both spouses must sign. This rule does not apply to spouses who will maintain separate addresses and filing statuses.
