Immunizations are used to protect children from illnesses such as chickenpox.

In Florida, a child's parent is required to submit his immunization records when enrolling him in school. A child may not be admitted without proof of immunizations. A student who is attending a Florida college and is residing on campus is also required to show proof she received a meningitis and hepatitis B immunization. If you are a resident of Naples and you are unable to locate your immunization record, you can request a copy of your records.

      Visit the Collier County Health Department main office. As of January 2011, the office was located at 3339 Tamiami Trail East, Suite 145 in Naples.

      Ask the clerk for a request form for your child's immunization records.

      Complete and submit the request form. You will need to provide your child's date of birth, name and birthplace on the form. You may be required to present identification when submitting the form.
