Use this kit to get a custom mold of your teeth and jaw and send it back to the company for them to custom mold the mouthpiece.

Instructions

1. Log onto the Internet and direct your browser to the Gladiator Mouthguard website. Select the sport, age and other options to see recommended mouthguards. Provide your address and contact information. Wait for the Gladiator Impression System to arrive in the mail.

2. Fill out the enclosed personal information forms with the impression kit.

3. Bite down on the enclosed foam blocks to make an impression of your teeth and jaw.

4. Place the completed forms and mold into the prepaid mailer included in the kit. Mail the package to Gladiator and wait for your custom mouthguard to arrive within two weeks.
