Start the process of changing your name in Florida online.

To change your legal name in almost any state requires a face-to-face hearing with a judge, but much of the documentation can be arranged online before you file. In Florida, you can fill out and print the name change request form. Then it's just a matter of filing that document in your area's circuit court and waiting for a hearing date. To qualify in Florida, you need to be 18, a current resident of the county in which your action will be filed and in full possession of your civil rights. Minors must have all legal guardians sign off on the change.

      Acquire your name change petition, available online from the Florida Courts website by navigating to the link in the Resources section. Click on the PDF download for the "Petition for Change of Name."

      Fill out the form in its entirety on your computer, adding information to all blank blue fields. When done, select "File," then save a copy of the PDF file to your computer. Then select "File" and "Print" the document. Sign it.

      Visit a notary public with two forms of identification to have your name change form notarized.

      Visit your local circuit court and ask for the civil department. Speak with a clerk and make sure you're filing your name change petition at the right courthouse. Pay any applicable filing fee (some courts charge these, ranging from about $10 and up), then wait.

      Attend your court hearing when alerted to it via mail. A judge will evaluate your case and make a decision. Whether granted a name change or not, you may be responsible for paying court costs.
