Accu-Chek provides diabetic patients with various products and tools to help manage the disease. Blood glucose monitors, testing strips and insulin pumps are just a few of the products offered by the Accu-Chek brand. Roche Diagnostics created the Accu-Chek Connect prescription discount card to help consumers afford testing supplies for diabetes management. The Accu-Chek Connect card can help lower your out-of-pocket costs for diabetic supplies, but you must follow important steps to activate your Connect card before use.

      Open the package for your Accu-Chek Connect card carefully. Look through the Connect card welcome package and read all of the information carefully to make certain you want to join the Accu-Chek Connect program. Read the terms and conditions on the back of your welcome page to fully understand how the Connect discount card works and what charges may be applied when purchasing diabetic supplies.

      Locate the sticker on the front of your Accu-Chek Connect prescription discount card. Do not remove the sticker until you are done activating the discount card. This sticker helps remind you to activate your Accu-Chek Connect card before using it to purchase diabetic supplies at your pharmacy or diabetic supply store.

      Call the Accu-Chek customer service number located on the sticker. An automated voice system will lead you through the activation of your Accu-Chek prescription discount card. Have your personal information ready, such as your full name, mailing address, phone number, Connect card identification number and medical insurance information if you have a medical insurance policy.

      Visit the Accu-Chek website address included with your Connect card welcome package, if you are more comfortable activating your card online. You will need to enter your full name, mailing address, phone number, Connect card identification number and medical insurance information when prompted. Click the blue "Activate Now" button located at the bottom of the activation page when you are finished entering all of the required information.

      Show your Accu-Chek prescription discount card to the pharmacist or diabetic supply company where you purchase your diabetic supplies. The Connect prescription discount will automatically be applied every time you present your Connect prescription discount card with a valid prescription for any Accu-Chek product. According to Accu-Chek, your activated prescription discount card ensures you are eligible for Connect program discounts and will pay no more than $15 for every Accu-Chek test strip prescription.

Tips & Warnings

  • Order your Accu-Chek Connect application tool kit online through the Accu-Chek Connect website.

  • The Accu-Chek prescription discount card is available only for consumers who don't have Medicaid or Medicare for prescription coverage. Attempting to use a Connect discount card without activating the card first may result in a denial of coverage at your pharmacy or diabetic supply store.
