Download an application for food stamps from the Illinois Department of Human Services website. You can download an application in Spanish if you prefer that to English. Links are provided in the Resources section.

Pick up an application at the Family Community Resource Center, 100 S. Martin Luther King Jr. Drive in Springfield, if you prefer. Telephone the office at 217-782-0400 if you need more information about how to apply.

Fill out the application as completely as you can. If you do not know all the information requested, you may leave some parts blank, but you must include your name, address and signature. Staff at the Family Community Resource Center will assist you with completing the application if you need help.

Provide proof of your identity (such as a driver's license), where you live and the Social Security numbers for all the people listed on your application. You may be asked to provide other documents as well. Turn in these documents along with your application. If any other documents are needed, you will be asked to bring them with you to an interview at the Family Community Resource Center.

Attend an interview at the Family Community Resource Center when requested. If you cannot go to the Family Community Resource Center for some reason, ask for a telephone interview instead. During the interview, a staff person will review your application and the documentation you provided. She will let you know how much assistance you will receive and when you will begin receiving it.
