A valid, goverment issued, photo I.D. is required to obtain birth records in New York State.

Two agencies supply certified copies of birth certificates to those born in the state of New York. Those born in New York City can obtain their birth record from the New York City Department of Health and Mental Hygiene. Those born outside of New York City can receive their birth certificate from the New York State Department of Health. Only the person named on the document, or their parent, is eligible to receive an official birth certificate.

  1. For Those Born in New York City

      Use a credit card, debit card or electronic check to order a certified copy of a New York City birth certificate. Orders placed online with be processed by the Vital Records Department within 24 hours. The cost is $15 for the birth certificate and $8.30 for service and mailing. Online orders can be placed at the website for the Office of Vital Records of the New York City Department of Health and Mental Hygiene.

      Apply in person at the Office of Vital Records location in Manhattan. Office hours are 9:30 a.m. to 3:30 p.m. Bring photo identification, address verification and a completed application (available from the website for the Office of Vital Records of the New York City Department of Health and Mental Hygiene.). Payment can be made using a credit or debit card, check or money order.

      Mail a completed application, copy of identification and address verification and appropriate payment to the Office of Vital Records for non-express birth certificate orders. Payment is $15 and can be made with check or money order made payable to the NYC Department of Health and Mental Hygiene. Include a self-addressed stamped envelope with your request.

    For Those Born Outside of New York City

Tips & Warnings

  • To avoid waiting in line and for quicker processing, it is recommended that you file your request for a certified birth certificate copy online. Most online orders can be processed within 10 days.

  • Remember when mailing your request that processing may take up to 30 days for requests in New York City and six to eight weeks for requests from the New York State Department of Health.
