How to Order FBI Fingerprint Cards

The Federal Bureau of Investigation (FBI) offers free fingerprint cards to anyone with an originating agency identification (ORI) number. The FBI issues several fingerprint cards that are used by agencies and law enforcement for a variety of purposes. Cards are issued with and without pre-printed ORI numbers. Once you receive your ORI number, the FBI offers an online system you can use to order different types of fingerprint cards.

Instructions

1. Call the FBI, CJIS Division at [phone] and request an ORI number. The FBI will provide ORI numbers to qualifying agencies, businesses and individuals.

2. Open a web browser on your computer and go to the the CJIS Fingerprint Supply Requisition Form (see Resources).

3. Complete the form by typing the requested information in the appropriate fields including your ORI number, contact information and the specific quantity of each type of fingerprint card you want to order.

4. Click the "Print" button at the bottom of the form.

5. Fax the form to [phone].

• You can check the status of your order by calling [phone]. Orders are processed in two to three business days, and orders are shipped using FedEx Ground.
