How to Get a Replacement Medical Card

The U.S. Department of Transportation requires individuals to pass a physical exam before licensure as commercial truck drivers. If a medical professional determines that the individual is healthy enough to drive a commercial vehicle, they will issue the driver a medical certificate, also known as a DOT "medical card." If the driver loses this card or certificate, replacement is usually a simple matter.

Request a Replacement

If you've lost your medical card, contact the medical professional who performed your physical exam and ask about her procedure for replacement. You may have to pay a fee for a replacement card or certificate. Another option is to ask your employer for a copy from your personnel file.
