   Provide narrative details. Reports must be detailed as possible to support the evidence of your claim. Present details of the accident or employment situation. Attach any additional documentation that will support your claim. Incident reports, witness testimonies and other facts should be added to your accord claim. Be sure to fill out all basic information such as your name, address and the damage amount you are seeking.
