With a little creativity and a few clicks of the mouse, you can create your own forms using Bento software for Mac OS X.

Bento is a personal database application developed by FileMaker Inc., for Mac OS X. People ranging from stay-at-home moms to business CEOs use Bento to organize contacts, track projects and plan events. Bento comes included with several customizable templates which also make it simple to create your own forms.

      Draw a diagram on paper of what you want the form to look like. It is not necessary to go into great detail, but have a general idea of what you want the form to look like to save time when actually creating it in Bento. For example, consider how many columns you want the form to have and where fields will be within those columns. Also decide how much spacing will be necessary between the various fields.

      Click on the Bento icon on the dock of your computer to open the program. If Bento is not kept in your dock, do a search for the program by typing "Bento" to the spotlight search tool in the upper right-hand corner of your screen (the magnifying glass icon).

      Create a new form by either clicking the "+" button in the upper right corner of the window; or selecting "Create New Form" from the "Forms" menu.

      Type in the name the form you have chosen. Use a title that is short but descriptive.

      Click on "Show Forms Tools" from the "View" menu located at the top of the screen.

      Drag fields into your form in correspondence with the plan you drew up earlier.

      Adjust the text and object size to suit the plan. Click on a field to adjust its size. White grab handles will appear that allow you to stretch or shrink the field to achieve the desired size. Sizes can be adjusted wider, taller or both. Notice the blue lines that act as guidelines for maintaining the same width for various fields. Follow them for a cleaner, more professional looking form.

      Click on "Save" if you wish to continue working on the form later or "Print" to print the finalized version. Both options are found in the "File" menu of Bento. If the latter is chosen, Bento will display a preview of the form you have just created. From this preview window you can adjust paper size and orientation if need be.
