Kentucky residents can apply for a new passport at various passport acceptance facilities in the state. Check with your local post office. Ask about handicap access or on-site photo service if you need it. As of November 2010, there is no Regional Passport Agency in Kentucky. If you will be traveling in less than two weeks or if you need your passport in four weeks to get a visa, you can schedule an appointment at a nearby Regional Passport Agency in another state.

Complete but do not sign Form DS-11: Application for a U.S. Passport. You can fill out the form online and print it out on one-sided pages or print the form blank and fill it out by hand.

Gather proof of your U.S. citizenship, two recent passport photos of yourself and photo identification, such as a driver's license or a military ID. Make a photocopy of your identification card. Your original ID and proof of citizenship will be returned to you.

Use the Bureau of Consular Affairs' Passport Acceptance Facility search page to find a passport acceptance facility in your city in Kentucky. Enter your zip code and click "Search" to find a list of nearby acceptance facilities.

Bring your passport application to the acceptance facility in person. For example, residents of Frankfort can apply at the Frankfort Post Office. Submit your identification card and its photocopy, your evidence of U.S. citizenship and passport photos. Pay the application fee. Your citizenship documents will be mailed to you along with your new passport. Sometimes the passport and documents are mailed separately.

Tip

  • The following can serve as primary evidence of U.S. citizenship: Certified official birth certificate (issued by state, county or city and not a hospital), naturalization certificate, previous and intact passport, citizenship certificate or a consular report of your birth overseas. Use secondary evidence of citizenship if you cannot obtain any of these.

Your 2-by-2-inch passport photos must be in color with a white background and taken in the past six months. You must be in normal street attire and facing forward in the pictures.

If you use your credit card as secondary identification, you are not required to provide a photocopy of it.

Additional requirements must be met for minor applicants.
