How to Create Custom Form Templates
A form template enables you to build a customized document that contains the form fields, styling and layout that you prefer. After you save your template, you have the option to use it again so that you do not have to repeat the design process. An Internet form builder is an ideal solution for creating customized form templates because you use the online tools to build your template and then have the ability to access your work again when you log in to your account.

Navigate to the JotForm website, click the "Themes" button in the menu bar and select a layout from the gallery that displays on the left side. Click the "Close Themes" tab button when finished to apply your choice.

Click a form element, such as "Heading," and double-click the item in the work area to enter your custom text. Continue to click the other available menus, such as "Quick Tools" and "Survey Tools," to add them to your form and personalize them.

Click the "Submit Form" button to generate your work and click the "Advanced Options" button. Click the "HTML Code" title on the left, then click the "Download Zip" link to obtain your form template. You also have the option to copy the URL address and code line that JotForm automatically displays to publish your customized form template to an email message or Web page.

Launch the Wufoo site and click the "Sign Up for Free!" button to generate a free form template. Follow the onscreen instructions to complete your registration, then click the "New Form" button when it appears on the page.

Click an a form option, such as "Multiple Choice," and double-click the element to type in your personalized text and set the width of the feature. Click the remaining options, such as "Paragraph Text" and "Email," to build your template, then click the "Save Form" button.

Click the "I'm finished! Take me back to the Form Manager" link to add your template to your account. Click the "Code" option that appears to get the information that allows you to publish your form. When you log in to your account again, click the "Forms" tab on top to get back to your template.

Bring up the EmailMeForm website and click the "Signup Now for FREE" button to register for a new account. Follow the prompts to complete your registration, then click the "Add Form" button that automatically displays on the page.

Click Option 2 in the display window that appears to specify that you want to create your own form from scratch. Continue to click the form menus, such as "Dropdown," in the "Add Field" section on the left and double-click each feature to enter the custom text that goes with the form fields.

Click the "Save Form" button at the bottom of the page, then click the "Take me back to the form manager" link that appears. Click the "Code" button to get the data that enables you to publish your form to the Web, or click the "My Forms" tab to access your form template when you log in again.

Tips & Warnings
To print your form template, click the "View" option offered on each website builder and use your browser's "Print" feature to obtain a hard copy of your document.
Each form provider sends you an email message when someone submits a form to you.
