Birth certificates are the legal documents that identify individuals when they are born. These records are kept by specific agencies within each state. Contact the specific agency to determine current fees for birth certificates. Submit a written request to the correct agency with the following information:
Full name of person on birth certificate
Gender
Parents names, including mothers maiden name
Month, day and year of birth
Purpose for copy of birth certificate
Relationship to the person
Daytime phone number

Write a check for the correct amount made out to the specific agency. Make copies of the letter and the check before mailing.
