1. Call The Scooter Store at the toll-free number 866-703-0345. A team professional who ask you several questions and fill out a mobility consultation form for you free by phone. You may also request to have a mobility consultation form sent to your address.

2. Wait while the paperwork, Medicare forms, and your insurance claim are filed and submitted for you.

3. Receive your scooter at home. It will be shipped to you upon acceptance of your claims.

USA Mobility, Inc.

1. Call USA Mobility, Inc., at 800-309-1667. You will speak with a team consultant, who can review your individual needs and determine what option suits you best.

2. Wait while your Medicare forms, insurance claims and paperwork are completed by USA Mobility, Inc.

3. Receive your new scooter chair. It will be shipped to you after your claim is accepted.

Tips & Warnings

• Medicare will pay 80 percent of your Scooter costs. You are responsible for the remaining 20 percent in out-of-pocket costs, unless you have supplemental insurance that covers the Scooter.
