How to Get My MVR Report From Alabama
A motor vehicle record (MVR) lists the driving habits of a car owner over a period of time. In Alabama, an MVR may be purchased by the driver of a car for the driver's own information, or it may be given to a third party such as an employer where the driver may become employed. Many insurance companies may also require a driver to provide a copy of her own MVR when insuring the vehicle.

- Check or money order for current application amount listed on application form
- Envelope and postage stamp

Download the recent Alabama MVR request application form at http://www.dps.state.al.us/driverlicense/forms/DLRecordRequest.pdf.

Print a copy of the Alabama MVR request application form and fill out the requested information. This will include the full name of the driver, the Alabama state driver's license number, Social Security number if the driver's license number isn't available, current address and a daytime phone number where the applicant may be reached.

Write a check or a money order made payable to "Alabama Department of Public Safety" in the amount listed on the application form. As of November 2010, the Alabama MVR request fee is $5.75.

Place the completed form and payment into an envelope and seal. Address the envelope to: Driving Record Request, Driver License Division, P.O. Box 1471, Montgomery, AL 36102-1471.

Tips & Warnings
Allow seven to 10 business days for a copy of an Alabama MVR report to arrive by mail after sending the request.
An MVR request form may also be physically obtained at any of the locations listed on the website at http://www.dps.state.al.us/driverlicense/reinstatement.aspx.
