      Visit the Palmetto GBA provider enrollment information page (see "Resources").

      Scroll down to the half-way point of the page and click on "Provider Information Enrollment Form." Double click on this link.

      Click on the "Railroad Medicare Provider Enrollment Information and Form" PDF link.

    • 4

      Print and complete the Railroad Medicare provider enrollment form and submit it to the address listed on the form.

Tips & Warnings

  • You must be a local Medicare provider before applying to become a Medicare Railroad provider.
