How to Find Medicare Railroad Application for a New Provider

Medicare Railroad is Medicare coverage for retired railroad employees and their beneficiaries.

Medicare and Medicare Railroad are often mistaken as being the same entity. Medicare and Medicare Railroad are two separate entities with different addresses for claims processing and different provider enrollment applications for new providers. Medicare Railroad recipients are retired railroad employees and their beneficiaries. Recipients of Medicare and Medicare Railroad receive the same benefits, but they are administered by different entities. Medicare Railroad makes it easy for potential new providers to apply by providing the application online.

Instructions

1. Visit the Palmetto GBA provider enrollment information page (see "Resources").

2. Scroll down to the half-way point of the page and click on "Provider Information Enrollment Form." Double click on this link.

3. Click on the "Railroad Medicare Provider Enrollment Information and Form" PDF link.

4. Print and complete the Railroad Medicare provider enrollment form and submit it to the address listed on the form.

Tips & Warnings

• You must be a local Medicare provider before applying to become a Medicare Railroad provider.
