How to File for a Hardship License in Missouri
If you've broken traffic laws or have more than one alcohol-related driving offense, you may have had your license suspended. In Missouri, you can apply for a hardship license (Limited Driving Privilege) to drive to work, school, to attend an alcohol education/treatment program, to seek the services of a certified Ignition Interlock Device provider, to seek medical treatment or other essential travel. Things You'll Need
- Missouri Department of Revenue Form 4595
- Proof of Insurance Form SR-22
- Proof of installation of an Ignition Interlock Device, for alcohol-related offenses

Have your insurance company file an SR-22 form showing that you carry liability insurance.

Have an Ignition Interlock Device installed on your vehicle if you have more than one alcohol offense on your driving record. The installer must be certified by the state of Missouri.

Complete the application for Limited Driving Privilege, Form 4595, and mail to: Missouri Department of Revenue, Driver License Bureau, P.O. Box 200, Jefferson City, MO 65105-0200 or fax to 573-522-8795.

Tips & Warnings
If your license has been suspended for five or 10 years, you must petition the court. File a petition for a Limited Driving Privilege with the associate court in the county where you live or work. This option requires you to pay filing fees and court costs.
You cannot get a Limited Driving Privilege to operate a commercial vehicle.
If your license has been revoked -- not suspended -- you cannot apply for a Limited Driving Privilege.
