Instructions

    • 1

Visit your local Ohio Department of Job and Family Services and request an application for public assistance. You can also call the office and have them mail you the form if you need to.

    • 2

Fill out the form. If you are confused about any questions on the form, you can call a case worker at the Department of Job and Family Services and they will help you.

    • 3

Review the Application/Reapplication Verification Request document to make sure you gather all of the appropriate documents needed to submit along with your application. This includes proof of citizenship, proof of social security number, proof of identity, proof of residency, proof of utility bill, proof of income, and proof of assets.

    • 4

Mail your completed application back to the Department of Job and Family Services or drop it off to the office. A case worker will then call you to make an appointment for a face-to-face interview.

    • 5

Bring the documents listed in the Application/Reapplication Verification Request to your scheduled appointment. The case worker will review your application to make sure it is complete and that you have all of the necessary documents. If you don't, you will be given some time to get them and turn them in.

    • 6

Wait to receive a letter of approval or denial from the Department of Job and Family Services. If you received a denial letter, you will be able to request a hearing to appeal the decision.

Tips & Warnings

  • Apply even if you think that you may not be eligible. The worst thing that can happen is that you are denied.
