2. Read the instruction booklet before beginning your application. Ask for help from the DHS if you cannot understand the information. The DHS is required to help those who cannot proceed on their own.

3. Fill out the assistance application and check the box next to "Food Assistance Program." Provide all of the information requested, including address, income and all other personal information. This must be provided for all members in the household.

4. Submit your application to the DHS in person, by mail or by fax. Include any proof of income or any other information that is requested. The DHS may also require an interview to determine your eligibility. Attend any scheduled interviews and bring any documentation that is requested.
