Choose "Birth Certificate," select the city you were born in, then click "Continue."

Select "My Birth Certificate." Fill out your first, middle and last name as it appears on your birth certificate, then click "Continue."

Enter your date of birth, then click "Continue."

Select the reason you'll be using your birth certificate, then click "Continue."

Choose which type of certificate you want--regular or expedited,--then click "Continue."

Fill out the final few steps, including parents' names and your gender, to confirm that your identity. Proceed to checkout and select the type of shipping you want.

Fill in your credit card information and billing address, and your birth certificate will be shipped to you.
