      Visit the Social Security website and download the application for a Social Security card (see Resources). Print and fill out the application.

      Use the Social Security office locator (see Resources) to find the nearest Social Security office.

      Bring the filled out application, proof of your United States citizenship and proof of your identity to your nearest Social Security office. Or, you may mail the application and documents to your local Social Security office. Your replacement card will be mailed to you.

  • Forms to prove citizenship are: birth certificate, consular report, passport, Certificate of Naturalization and a Certificate of Citizenship.

  • Forms to prove identity are: driver's license, state-issued identification card and a passport.
