How to Nominate Someone for an Extreme Home Makeover
"Extreme Home Makeover" show is a reality show on ABC where a real family in need gets its home upgraded. The families chosen by Lock and Key Productions typically have an inspirational story. The company says it chooses the families that seem most deserving based on applications sent to the show. Lock and Key Productions receives thousands of applications a week so you must complete all the application requirements thoroughly or your nomination is not likely to be considered.

Go to the ABC.com website and click on the "Extreme Home Makeover" show main page. The application form can be downloaded from the show's page.

Fill out the family's information on the application form. You'll need to include all of the members living in the household and their age, date of birth, occupation or education level.

Answer the questions about the family including listing web addresses to any social media for any of the members. You'll also have to list the household income, work information, state aid benefits, any criminal convictions, volunteer or community experience, debt amounts, car types, military experience and more. If you are not submitting your own family, and you do not have all of this information on hand, just fill in what you can.

Summarize the family's current situation and why it deserves the home makeover. It must also list the top five repairs the home needs immediately. The application also calls for specific information about the style of the house, the number of levels, bedrooms and bathrooms, its lot size and its square footage.

Sign the eligibility requirement, consents and releases. A shooting location agreement will also need to be signed. All of the required paperwork is in the application.

Create a family casting video on a DVD, MINI DV, VHS or MINI DVD format. The casting video must include a tour of the home and should include all members of the household. They all need to speak at least once in the video. It must include a family greeting shot, a family goodbye shot and interviews.

Take a photo of the family and all household members. Submit this photo along with photos of the house, the application and video to Lock and Key Productions. Applications are mailed to:
