How Do You Change Your Name & Address for Voting in New York?
In New York State, residents who change their name or address are required to submit this change in writing. This can be done on a Voter Registration Form. The Board of Elections will then cross-check the new name or address with records from the U.S. Postal Service, the Department of Motor Vehicles, and/or the U.S. Social Security Administration.

Obtain a voter registration form. This can be done on the Board of Elections Web site (reference 1), at your local DMV or Board of Elections Office, or by writing to the Board of Elections.

Fill out the Voter Registration Form as if you were registering for the first time. In Section 11 of the Voter Registration Form, enter the information that has changed.

Return the Voter Registration Form with your current information by mailing it to your county Board of Elections address which can be found on the Voter Registration Form or in person at the DMV or Board of Elections office.
