When you apply for Medicaid in Colorado, you will fill out a general application and submit it to the Colorado Department of Health Care Policy and Financing. This application can be used to apply for both Medicaid and CHP+, the state's health insurance program for children. Colorado Springs is located in El Paso County and, according to Peak Vista Community Health Center's website, there is an abundance of people in the county who are eligible for Medicaid, but who are not covered by Medicaid. Peak Vista has developed a Medicaid application website to assist El Paso County residents with the Medicaid application process.

      Download and complete the Medicaid application form available at Peak Vista's Medicaid application website. You can either print the application and fill it out by hand, or you can fill in the forms by typing into the form's fields and print it out. You can pick up a Medicaid application at Peak Vista's Registration site at 2840 International Circle in Colorado Springs, or from the Department of Human Services. The Department of Human Services has several locations, and you can find them at the department's website at http://dhs.elpasoco.com/.

      Gather identification and proof of residence, as well as financial records of everyone for whom you are applying for Medicaid. If you or those you are applying for have a Social Security number, you will need to submit proof of it/them as well. Turn in copies of your household's income such as pay stubs and child support, as well as bank account information. You will also submit expense-related information such as for rent and day care, your health insurance card (if applicable), and a letter certifying a baby's due date if you are pregnant. Peak Vista's website has a list of the various application assistance sites available at http://www.peakvista.org/images/pdfs/Application_Assistance_Sites.pdf. Personnel at these sites will help you complete your application, ensuring you have supporting material that meets requirements and answering technical questions regarding the application process. There are seven such sites available in El Paso and Teller Counties. Bring your application and supporting materials such as financial records and identification with you when you go.
