How to Make a Sign Up Form
There are many activities where you could use a sign-up form. A sign-up form is quick and easy and can be done in a variety of software applications, including a word processor like Microsoft Word or a spreadsheet application like Microsoft Excel. The information on a sign-up form is pretty simple. You will want the name, address, phone number and perhaps the email address of each of the people who will sign your form. You can add additional columns as needed so that your form fits your specific requirements.

Using a Word Processing Program with Table Capabilities

Type the purpose of your sign-up form at the top center of the paper. Set your page layout for landscape rather than portrait if you will need more than four columns.

Space down two lines and set your orientation for left alignment and at least double spacing using your formatting tool bar.

Insert a table into your document. Click on the "Insert Table" icon in your "Formatting" or "Tables and Borders" toolbar and choose the number of columns and rows you will need in your table. You will need one column for each piece of information (name, address, etc...), one row for your column headers and a row for at least as many people as you hope will sign up.

Select "AutoFit" for column widths. You can adjust the column widths after your table is inserted. Click "OK," and MS Word will insert your table into your document. Your table will already include grid lines.

Type your column headers in the top row. Select Bold so your headers will stand out.

Position your cursor over any column divider you want to change and drag it so that your column has the dimension that seems appropriate to you.

Set row widths to at least double spacing.

Save your document and print.
Using a Spreadsheet Program Like MS Excel

Open your workbook. Set your page set-up orientation for landscape.

Type your column headings in your top row, using whatever formatting seems to be appropriate.
Adjust your column widths to accommodate the information size you need by placing your cursor over the column divider and drag the line to whatever dimension seems appropriate.

Drag the cursor to select as many lines and columns as your sign-up form needs and select your borders option. Select the option for lines between each column and each row.
Change the line spacing to at least double-spacing.

Double-click above your spreadsheet grid and add a document header if you choose.

Save and print your document.

Tips & Warnings
Use landscape orientation for sign-up sheets requiring more than 4 columns.
Ask people to print legibly on the form. You can add that request to the page footer.
