How do I Find Instructions to Fill Out the FMLA Application?
The Family Medical Leave Act guarantees workers leave of absence to deal with medical emergencies involving the employee or a family member without fear of losing their job. The FMLA requires employers to offer time off to eligible employees for medical or other family reasons and to allow them to return to their jobs at the end of the FMLA leave with no reduction in pay or benefits.
According to the U.S. Department of Labor, eligible employees are those who have worked for more than 12 months and worked at least 1,250 hours during the preceding year.

Read the Department of Labor FMLA notice. By law, your employer is required to post notices informing employees of their rights regarding fair wages, taxation and leave of absence. The Department of Labor publishes a poster describing your rights and responsibilities under the Family Medical Leave Act of 1993. This poster will be prominently displayed with the other required notices in an area of the workplace such as an employee break room or locker room.

Make an appointment to talk with someone in the company Human Resources department about taking FMLA leave. For foreseeable situations covered by the FMLA you are required to request leave 30 days in advance. The HR person can help you submit your request and fill out the FMLA application

Look in your company employee handbook to find your options for taking leave. FMLA leave is unpaid leave and many companies will allow you to use accrued sick leave or vacation time in conjunction with FMLA leave.

Check your company website or intranet for more information. Most companies are using company networks called intranets to publish information and to communicate between employees and management. Your company will have information regarding the FMLA application and taking FMLA leave available to you on its intranet or Internet website.
