How do I Fill Out a Lions Club Certificate?
Lions Club International is a service organization that operates in over 206 countries and has more than 1.35 million members. The Lions Club performs community service, awards grants and provides volunteer opportunities for youth. In the event of bodily injury or property damage that results from Lions Club activities or functions, Lions Club International provides a general liability insurance that covers members. The form for the Lions Club certificate of insurance is located on their website. Members can create their own certificates by filling out this form.

Go to the Lions Club International page with certificates of insurance (see Resources).

Select which type of certificate you need to provide proof of insurance. There are three options: 1) "This form is used to show proof of insurance. No additional insured is needed." 2) "This form is used to show proof of insurance and to add additional insured's for use of their premises." 3) "This form is used to show proof of insurance and to add additional insured's--State or Political Subdivision--for issuance of permits."

Fill out the name and date of the event for which the insurance is needed.

Input your phone number, club or district name and where the event will be held.

Fill out the certificate holder information section. This section is the information of the person, organization or business to which you're providing the certificate of insurance. Fill out the name, address, city, territory and country and ZIP code fields.

Click the submit button, which will take you to next page with a link for your .pdf file.

Click on the link for the .pdf file, which will bring up the certificate of liability of insurance certificate.

Print the certificate.
