How to Enroll for an NPI Number
National Provider Identifier (NPI) numbers are issued by the National Plan and Provider Enumeration System to health care providers. According to the U.S. Department of Health and Human Services Centers for Medicare and Medicaid Services website, NPI numbers are used to identify health care providers in standard health care transactions, prescriptions and medical system files and records. You can enroll for an NPI number using the application form on the NPPES website.

Go to the National Plan and Provider Enumeration System NPI Application Instructions website (see Resources).

Gather the personal information of the individual provider if you are applying for an NPI number as an individual health care provider. If you are applying for an NPI number as an organization, you will need the organization's Employer Identification Number, address and contact information.

Read the information listed in "Step 2" of the "NPI Application Information" article. Click "Begin Application Form" if you agree to these terms and conditions.
