How do I Get a New Card for Keystone Health Plan East?

You can still receive health benefits without your official identification card.

Customers of Keystone Health Plan East--a health maintenance organization (HMO) insurance plan under Independence Blue Cross--need to show their identification cards to receive health services or fill prescriptions from plan providers. If you lose your Keystone identification card, you can get a replacement within 30 days. You can also request your card if you do not receive it within 30 days of enrolling.

Instructions

1. Collect the personal information of the card holder, including full name, date of birth, Social Security number and employer.

2. Call or write to Keystone, provide all your information from Step 1, and explain that you need replacement cards or that you never received a card after enrolling. Or go into the member's area of the Independence Blue Cross website, and fill out the form for getting a replacement card.

3. Contact Keystone again if you do not receive your card within 30 days.

Tips & Warnings

• You can still receive benefits without your official identification card. Until you receive your card, use a copy of your Health Benefits Election Form (SF-2809), your health benefits enrollment confirmation or your electronic enrollment system confirmation letter as proof of your enrollment.
