How do I Get a Sprint Claim Form From the Internet?

Customers pay for their insurance monthly, as part of their Sprint bill.

Sprint is a cell phone service provider that offers insurance on its devices. The insurance, which is through the company Asurion, covers theft and damage. When something happens to a policy holder's cell phone, he must fill out a claim form and send it to the company, who will then send them a replacement device. You can find the claim forms online.

Instructions

1. Open the web browser of your computer. Visit the Asurion's website for downloading claim forms (see Resources).

2. Click the blue "Download Form" button and print out the form. Fill it out entirely.

3. Fax the completed claim form to Asurion at 1-888-832-0511.
