Enter your first and last name, address and email information. Input your birthday and due date. Select the appropriate responses to the remaining questions. Create a user name and password, security question and input any promotional code you may have.

Fill out the appropriate family information and click "Submit." Select a user name and password. Choose your security questions and enter the proper responses.
