A career as an Occupational Therapist can include work in rehabilitation centers, schools, homes, adult day care programs, hospitals and nursing care facilities. In Florida, it is required that you have graduated from an accredited O/T program, completed a minimum of six months field experience, completed a two-hour pre-approved Board of Occupation Therapy practice provider educational course on prevention of medical errors and passed the examination before receiving a license.

      Complete the application form located at the web site doh.state.fl.us. Click on the applicable application link to download the proper application. All questions on the application must be answered. Enter N/A in any field if the question does not apply.

      Print your name on the back of a passport style photograph. The photograph should be approximately 2" x 2 1/2" in size, showing your shoulders and head only and taken withing the last six months. Attach the photograph to the application.

      Make a check, certified check or money order payable to DOH/Board of Occupational Therapy Practice in the amount of $180. The total cost includes $100 non-refundable application processing fee, $75 initial licensure fee and $5 unlicensed activity fee.

      Mail your application and fees to: Florida Department of Health, Occupational Therapy Board, PO Box 6330, Tallahassee, FL 32314-6330.
